FACE LIFT AND NECK LIFT CASE #361

A 49-year-old cosmetic patient from Fort Lauderdale came to our plastic surgery practice for consultation about facial rejuvenation. She was concerned about hollowness around eyes, flat, droopy cheeks, skin folds on upper eyelids, small jowls and early signs of skin laxity in the neck.

After a detailed examination and extensive discussion about available surgical and non-surgical options, we decided to proceed with upper and lower eyelid surgery, face and neck lift and fat grafting of multiple areas of the face.

Surgery was done under general anesthesia and lasted 6 hours. Fat was harvested from her flanks and prepared for fat grafting. Using a small 1 ml. cannula, the fat grafts were injected into her cheeks, temples, around eyes, eyebrows, nasolabial crease, around mouth, along jawline and chin. Upper blepharoplasty was done with conservative excision of eyelid skin to avoid sunken eye appearance. Lower blepharoplasty was performed by means of transconjunctival blepharoplasty. The face and neck lift with SMAS plication followed. The patient was released home 1.5 hours after surgery. A small drain was removed from her neck the next day; she was allowed to shower and wash her hair the day after surgery. Mild swelling (due to fat grafts mostly) persisted for 2-3 weeks. She was back to work in 3 weeks.

The patient is shown before facial rejuvenation procedures and 1 year after upper and lower eyelid surgery, fat grafts for volume restoration in multiple areas of the face and face and neck lift with SMAS.
