This patient  presented for breast augmentation with implants. Her bra cup size was 32A; she wanted augmentation with silicone implants which will give her two cup sizes up, 32 C/D. After thorough evaluation and measuring with implant sizers and 3D stimulation, she decided to have smooth, silicone gel, round implants with volume of 350 ml.. Surgery was done in general anesthesia;  5 cm. incision in the lower pole/breast crease (inframammary fold) was used to place implant underneath the muscles. Series of intercostal nerve blocks was given to provide pain free postoperative course. Patient is presented before surgery and 6 months after breast augmentation with smooth, round, silicone gel implants of 350 ml..
