A 54-year-old patient came to our plastic surgery office in Fort Lauderdale for consultation regarding rejuvenation of cheeks and eyes. She stated that she had hollow, flat cheeks for her entire life but it became more obvious with aging and after she lost weight. She claims that she looks tired and sad.

After clinical evaluation we discussed available surgical options to correct underlying deformity mostly due to weak, poorly developed cheek bones (malar hypoplasia), deflated skin envelope and excessive skin folds over upper eyelids. We decided to proceed with surgery which will include cheek augmentation with silicone implants, upper and lower eyelid surgery and fat grafting.

Surgery was done in twilight anesthesia in our plastic surgery center in Fort Lauderdale. It included harvest and transfer of fat grafts to cheeks, temples, around eyes and eyebrows, upper blepharoplasty and transconjuctival lower blepharoplasty. Cheek implants were placed through lower eyelid incisions.

Patient is shown before eyelid and cheek surgery; notice flat ,deflated cheeks with droopy skin folds, skin folding over upper eyelids and tired, sad look. Postoperative photos, done 10 months after upper and lower eyelid surgery, fat grafting, lateral canthopexy (tightening of lower eyelid) and cheek augmentation with implants show significant improvement, fuller cheeks, loss of cheek folds and softer, natural look. Further fat grafting of the cheeks and additional removal of upper eyelid folds is planned.
