MALE BREAST SURGERY - GYNECOMASTIA CASE 144

A 57-yer-old out-of-town plastic surgery patient came for consultation regarding male breast surgery. Several years ago this very active and athletic patient noticed that his breasts became larger and saggy. He wanted to see how this deformity can be improved surgically with minimal downtime. He was not interested in “perfect result” but he wonted visible reduction in size of his breasts.

After clinical evaluation in our plastic surgery center in Ft. Lauderdale we discussed surgical options for treatment of male breast enlargement (gynecomastia). We suggested removal of fat and breast tissue by means of liposuction and removal of skin around and bellow areola to tighten skin envelope. He opted for breast liposuction without skin removal.

Surgery was done in our office based plastic surgery center in general anesthesia. Approximately 200 ml of fat and breast tissue was removed from each breast with liposuction cannula. No skin was removed. Tight postoperative dressing and garment were applied and patient was discharged to his hotel with his wife as “a care provide”. His recovery was uneventful except for general soreness in the treated areas for 3-4 days. He went to work (he is gynecological surgeon) 7 days after surgery.

On the left patient is shown before gynecomastia surgery; postoperative images on the right show result 6 months after aggressive lipectomy (liposuction) of the breast tissue.  There is still mild sagging of the breast skin envelope which could have been avoided provided that patient accepted initially proposed skin reduction (excision of small area of skin around and bellow areola).
