BLEPHAROPLASTY CASE

A 46-year-old Palm Beach cosmetic patient came to our plastic surgery center in Fort Lauderdale for consultation regarding appearance of eyelids and brows.  She wanted to have her eyes “more open” and “lower eyelid bags and wrinkles removed”.

On clinical examination we found that she has moderate bulges of the fat compartments in the lower eyelid as well as a moderate groove so called “a tear trough deformity” between cheek and lower eyelid. She also had a mild redundancy (wrinkling) of the lower eyelid skin (left).

We decided to perform her lower blepharoplasty using incision inside her lower eyelid. Redundant fat tissue was partially removed and partially distributed underneath the tear trough groove (nasojugal groove) to eliminate the groove (transconjuctival blepharoplasty). Minimal removal of the lower eyelid skin was done using incision hidden just below eyelashes. Patient underwent an endoscopic brow lift in the same time which significantly improved position and symmetry of her brows, “opened her eyes”  and eliminated a need for upper eyelid surgery (upper blepharoplasty) (right).

Before lower blepharoplasty (left) and 6 months after transconjuctival lower blepharoplasty (right).
